How a treatment is chosen

There is a tendency to think of fertility treatment as a ladder you climb — IUI, then IVF, then ICSI. In practice the choice depends on what the investigations show.

If ovulation is the problem, ovulation induction may be all that is needed. If the tubes are blocked, IUI cannot work and IVF is the route. If sperm count or motility is very low, ICSI addresses that directly. If a woman is 40, spending a year on treatments with low prospects carries a real cost in time. Each of these is a different answer to a different question.

What this means in practice is that the assessment matters more than the menu. Your specialist will explain why a particular approach is being suggested for you, what its realistic prospects are, and what the alternatives would be.
