Information we collect
We collect information you give us directly — your name, contact details, date of birth, medical and reproductive history, and details of previous treatment. We also generate clinical information in the course of your care: investigation results, scan reports, laboratory and embryology records, and treatment notes.
If you contact us through this website or by email, we hold the contact details and the content of your message.
How we use it
- To provide clinical care and to plan and deliver your treatment
- To contact you about appointments, results and follow-up
- To maintain the medical and laboratory records we are required to keep
- To meet legal and regulatory obligations, including those under the ART (Regulation) Act, 2021 and the PCPNDT Act, 1994
- For billing and administration
Confidentiality and disclosure
Your medical information is confidential. We do not disclose it to anyone outside your care team without your consent, except where we are required to by law or by a regulatory authority, or where disclosure is necessary to prevent serious harm.
Where treatment involves both partners, information relevant to the joint treatment is shared between you. Consent for the storage and use of gametes and embryos is recorded separately for each partner, as the law requires.
Storage and security
Records are held securely, with access limited to staff who need it for your care. Clinical and ART records are retained for the periods required by law.
Your rights
You may ask to see the personal information we hold about you, ask us to correct anything inaccurate, and ask questions about how your information is used. Requests can be made in writing to the centre or by email to sreenandakafertility@gmail.com.
Cookies and this website
This website does not set advertising or tracking cookies. Fonts are loaded from Google Fonts, and the online booking link takes you to a third-party appointment service which operates under its own privacy policy.
